Poze 15mg Tablet 14 'S is an oral antidiabetic medication that is used to manage type 2 diabetes. Type-2 diabetes is non-insulin-dependent diabetes in which the insulin produced by the pancreas is less effective because the body doesn’t recognize it. Poze 15mg Tablet 14 'S being an oral antidiabetic agent, decreases insulin resistance and renders the body to make beneficial use of insulin in controlling diabetes. 

Poze 15mg Tablet 14 'S can be worsening in people with heart failure. Tell your doctor if you had congestive or any other type of heart failure in the past before starting the treatment with Poze 15mg Tablet 14 'S. Indications

Poze 15mg Tablet 14 'S is indicated in the individuals with type 2 diabetes as: 

  • An adjunct to exercise and diet to enhance the body’s glycemic control 
  • Monotherapy, specifically in overweight patients in which exercise and diet are not enough. Also, to whom Metformin is contraindicated or intolerant?
  • Combination with metformin (dual therapy) for glycemic control or with sulfonylureas if Metformin is contraindicated or intolerant. 
  • Triple therapy in combination with Sulphonylureas and Metformin in overweight patients who have insufficient glycemic control even despite the dual combination therapy with oral antidiabetic agents.  
  • Combination with insulin when glycemic control is not sufficient in patients who are on insulin. 

How Does Poze 15mg Tablet 14 'S Work?

Poze 15mg Tablet 14 'S contains the active ingredient Pioglitazone which is responsible for causing insulin tissue sensitivity. Adipose tissue, skeletal muscles, and liver are the target sites for insulin’s action. These sites have Peroxisome proliferator-activated receptor gamma (PPARγ) that is involved in the insulin response. Poze 15mg Tablet 14 'S acts as a selective agonist to peroxisome proliferator-activated receptor gamma (PPARγ) at these insulin-acting sites and increases insulin-responsive genes transcription.

These insulin-responsive genes are involved in controlling the production, transport, and utilization of lipids and glucose. This way, the active ingredient - pioglitazone increases tissue sensitivity and decreases the hepatic production of glucose. This improves type 2 diabetes-associated insulin resistance without abnormal insulin secretion by the beta cells of the pancreas. 

How To Use Poze 15mg Tablet 14 'S?

Poze 15mg Tablet 14 'S is either used alone or in combination with other antidiabetic agents such as metformin, glyburide, and sulfonylureas. Poze 15mg Tablet 14 'S should be taken orally (by mouth) without regard to a meal.

Dosage

The active ingredient - Pioglitazone comes as an insulin sensitizer with the following strengths:

  • 15mg tablet 
  • 30mg tablet
  • 45mg tablet 

Adult Dose

Type 2 Diabetes Mellitus

  • As monotherapy or in combination with insulin or insulin secretagogue: 15 to 30 mg once daily with a meal. The dose can be increased by 15 mg with careful monitoring but should not exceed more than 45mg once a day (whether it's monotherapy or combination therapy). ALT (liver enzyme) should be monitored upon initiation of the therapy. The treatment continues once a month for 12 months, and then after every three months. 

The dose of insulin is decreased by 10 to 25% when administered with insulin if the patient has hypoglycemia or if the blood glucose levels go below 100mg/dL. The dose of insulin secretagogue (e.g. metformin, sulfonylurea) is decreased when administered in combination with these agents if the patient reports hypoglycemia. 

Pediatric Dose

Poze 15mg Tablet 14 'S is not indicated for use in children.

Pregnancy And Lactation

The mechanism of action of Poze 15mg Tablet 14 'S is not clear in pregnant women, and neither the active component is known to be secreted in human milk. Therefore, Poze 15mg Tablet 14 'S is not recommended for use in pregnant and lactating females. 

Contraindications

Poze 15mg Tablet 14 'S is contraindicated in the following cases:

  • Hypersensitivity to the active agent or excipients. 
  • Symptomatic cardiac failure or history of any type of cardiac failure.
  • Moderate to severe liver Impairment 
  • Diabetic Ketoacidosis (lack of insulin to allow the entry of blood sugar into the cells to use energy)
  • Bladder cancer or active renal symptoms (e.g., blood in urine) 
  • Active osteoporosis or osteopenia (bone diseases) 

Precautions

Caution should be taken with the use of Poze 15mg Tablet 14 'S in the following cases:

  • Poze 15mg Tablet 14 'S puts a person at risk of congestive heart failure upon the initiation of therapy or after increasing the doses. The patient should be closely observed for the symptoms of heart failure, such as rapid weight gain, edema, shortness of breath, etc. Dose reduction or discontinuation of therapy with Poze 15mg Tablet 14 'S should be considered if these symptoms are visible. 
  • As Poze 15mg Tablet 14 'S its insulin-sensitizing effect only in the presence of insulin, it is not for use in people with type 1 diabetes or diabetic ketoacidosis.
  • If you are taking Poze 15mg Tablet 14 'S in combination with other antidiabetic agents, their dose reduction is necessary as administering the combination may put you at risk of hypoglycemia. 
  • Poze 15mg Tablet 14 'S has the potential to decrease the levels of hemoglobin and hematocrit. So routine monitoring of hematocrit helps assess present side effects. 
  • People with active liver disease or elevated liver enzymes should not use Poze 15mg Tablet 14 'S as it tends to elevate liver enzymes.
  • People who have mild elevation in their liver enzymes should initiate or continue the treatment with Poze 15mg Tablet 14 'S cautiously with routine monitoring and clinical follow-ups. 
  • Poze 15mg Tablet 14 'S has the potential to cause weight gain, but it is dose-related. Dietary changes would help to cope with obesity. 
  • Poze 15mg Tablet 14 'S has the potential to cause blurred vision and macular edema (building up of fluid in the macula of the eye). Routine examination with the ophthalmologist is advised. 
  • Females who are on Poze 15mg Tablet 14 'S are more vulnerable to the risk of fractures. 
  • Geriatrics are at increased risk of the incidence of bone fracture and heart failure. 

Interactions With Other Medications

Following medications interfere with the metabolism of Poze 15mg Tablet 14 'S and show interaction:

  • Rifampicin
  • Midazolam
  • Baricitinib
  • Gemfibrozil
  • Ethanol
  • Eluxadoline

Your healthcare provider must know if you are on these drugs or have recently consumed them before the initiation of therapy.

How To Cope with Some of The Adverse Effects

  • Hypoglycemia- You can cope with the symptoms of hypoglycemia by taking 15 to 20 g of fast-acting carbohydrates. Fast-acting carbohydrates have sugar content without protein and fat, so they are easily converted into sugar, once inside your body. Stay in a resting position and drink fruit juice, and glucose tablets to help cope with low blood sugar. 
  • Weight gain - Weight gain can be controlled by lowering the dose of medicine, eating in little portions, avoiding high calories alcoholic beverages, increasing physical activity such as walking & exercise, and increasing protein intake. Protein will make you feel fuller for a long time. You can also limit weight gain by limiting salt intake i.e., consumption of fewer than 2000 mg of salt per day.
  • Anemia - Pale skin, fatigue, weakness, and disturbed appetite are signs of anemia. Eat iron-rich food such as beef, salmon, cereals, liver, dry fruits, and leafy vegetables. Take a good night's sleep, take rest when needed to avoid fainting, and light exercise daily improves blood flow. 
  • Dizziness - Do not get up or change your position immediately when you are feeling dizzy. Instead, get up slowly and steadily. Increase your fluid intake to stay energetic, and refrain from coffee, smoking, alcohol, and addicting drugs.
